Using a key-worker system and holistic 1-1 support, Aftercare provides a range of practical and emotional support interventions to recovering substance misusers who are substance free, aiming to enable individuals to re-establish themselves in their communities without relapse to substance misuse and/or crime.Referral Criteria
Clients who are:
- Recently Drug or alcohol free.
- Over 18 years of age.
- Requiring support to remain substance free.
Referrals are welcome from a range of agencies including people leaving custody and self-referrals.
